Cornerstone Family Practice offers an integrated model of care, with attention to both physical and mental well-being for your overall wellness. Integrated care allows behavioral concerns identified by your primary care provider to be addressed right away. Conveniently located in the Guttenberg clinic, our behavioral health team works closely with your medical provider and other professionals to deliver the best overall health care.
Our staff of Psychiatric Nurse Practitioners and Licensed Independent Social Workers have schedules that allow you to receive care when you need it. Both in-person and telehealth appointment options are available.

If you have questions about behavioral health care, or think you (or someone you know) might need a behavioral healthcare appointment, talk to your primary care provider.
