Integrated Medical and Behavioral Health
Caring about your mental health is as important as maintaining your physical health.
Cornerstone Family Practice offers an integrated model of care, with attention to both physical and mental well-being for your overall wellness. Integrated care allows behavioral concerns identified by your primary care provider to be addressed right away. They can write a referral for you to be seen by one of our behavioral health providers. Both in-person and Telehealth options are available.
Appointments can be made by calling 563-252-2141.
